Cancel my subscription

Can someone help me here I would like to cancel my subscription. I sent one of the admin a message but couldn't get any reply last month and I really don't want to keep paying for something I don't use at all.

For data matching I also need your date of birth and the name of the bank we have on file. 
If you have specified PayPal or transfer yourself, your address. 
If you pay by credit card, please enter the last four digits of your credit card number.
